Our Nurse Health Case Management services ensure that all aspects of an individual’s healthcare are coordinated, monitored, and continuously improved. We provide:

Nurse Health Case Management
Our Nurse Health Case Management services ensure that all aspects of an individual’s healthcare are coordinated, monitored, and continuously improved. We provide:
- Comprehensive nursing assessments of health status, medical needs, and service requirements
- Completion and ongoing review of the Health Risk Screening Tool (HRST)
- Completion and review of the DDA Medication Administration Screening Tool (initial and upon changes in health status)
- Coordination of medical appointments, follow-ups, and provider communication
- Development and implementation of individualized health care plans and protocols
- Ongoing monitoring of health services delivered by DDA providers and other care team members
- Identification of changes in health status and timely updates to care plans

Nurse Case Management & Delegation
Our Nurse Case Management and Delegation services ensure that care is delivered safely, effectively, and in full compliance with regulatory standards.We provide:
- Ongoing review and management of each individual’s healthcare plan
- Assessment of individual needs, abilities, and health-related risks
- Evaluation of caregiver performance and competency in delivering care
- Assessment of the service delivery environment to ensure safety and appropriateness
- Training, supervision, and delegation of nursing tasks to unlicensed direct care staff in accordance with Maryland Board of Nursing (MBON) regulations
- Continuous oversight of delegated tasks to ensure accuracy, safety, and compliance
- Clear communication and guidance to individuals, families, and caregivers to support quality care outcomes
- 24/7 on-call nursing support for staff performing delegated nursing services
